Our Pathways

1. Thriving & Stability Pathway

For individuals stepping down from inpatient care and preparing for independent living.

 

This pathway is designed for people who have completed an inpatient admission and are ready to transition back into the community with structured support. The focus is on maintaining progress, preventing relapse, and building confidence in everyday life.

 

Core Purpose

  • Provide a safe, supported step-down from hospital.

  • Strengthen recovery skills in a real-world environment.

  • Help individuals gradually reintroduce community routines and responsibilities.

  • Prevent relapse by offering consistent, specialist input.

How It Works

A specialist Occupational Therapist completes a detailed assessment and co-develops a personalised plan.

Support can take place in the person’s own home or in helping them find suitable accommodation.

The OT coordinates input from support workers, nurses, and assistant psychologists.

  • Meal planning, preparation, and building confidence with food.

  • Returning to education, work, or volunteering.

  • Developing routines, structure, and independence.

  • Building social confidence and community participation.

  • Managing wellbeing, coping strategies, and relapse prevention.

2. Empowered Recovery Pathway

For individuals needing 24/7 supported accommodation to stabilise and strengthen recovery

 

This pathway offers a more intensive level of support within one of the service’s supported living homes. It is suitable for people who are not yet ready to live independently but do not require acute inpatient admission.

 

Core Purpose

  • Provide a safe, therapeutic living environment with round-the-clock support.

  • Reduce the need for hospital admission.

  • Help individuals stabilise, build skills, and progress toward greater independence.

How It Works

Residents live in one of the service’s supported accommodation houses (Trafalgar House or White House).

Staff are available 24 hours a day, including overnight.

The programme mirrors the Thriving & Stability Pathway but is delivered in-house with continuous support.

Areas of Support

  • Mealtime support and food-related exposure work.

  • Daily living skills (personal care, cleaning, laundry, budgeting).

  • Social eating, community outings, and graded exposure.

  • Emotional regulation, DBT skills, and coping strategies.

  • Identity-building, meaningful activity, and role development.

  • Healthy movement and body awareness sessions.

  • Structured routines, goal setting, and weekly reviews.

Ideal For

  • Individuals who cannot yet manage safely or effectively in the community alone.

  • Those stepping down from inpatient care who still require high-level support.

  • People who need a therapeutic environment to consolidate recovery.

Trafalgar House, Milton Keynes – 4 bedrooms (2 en-suite, 2 shared bathrooms)

White House, Milton Keynes – 4 bedrooms (2 en-suite, 2 shared bathrooms)

Both homes offer spacious bedrooms, communal lounges, dining areas, and gardens.

New developments in London and surrounding areas, Bracknell, Aylesbury and Huddersfield.

Our dad's time was coming to an end, and we wanted him home in his house from the hospital. [Hillingdon Hospital was great.] Home care was needed.4319 fountain care was arranged, and the best thing that could have happened, the whole team was absolutely bang on, went well above what they had to do, really knew their job, as we know it's not easy. They always came, never missed, and were genuinely upset when Dad's end came. Not going to name separate people as they work as a team and are all brilliant. We visited their office a year later just to say we had not forgotten what they did for dad and us. Thank you from all of us, Joe & all of the family.
